Liability Waiver for Vacation Bible School (VBS)

I hereby acknowledge that I have voluntarily chosen to participate in the Vacation Bible School (VBS) organized by Philippi Church of Christ Disciples of Christ. In consideration of being allowed to participate in this event, I agree to the following terms and conditions: Assumption of Risk: I understand that participating in VBS may involve certain risks, including but not limited to, physical injury, accidents, and the transmission of communicable diseases. I voluntarily assume all such risks and accept full responsibility for any personal injury, loss, or damage that I may sustain while attending VBS. Medical Attention: In the event of an accident or medical emergency, I hereby authorize the VBS staff and volunteers to secure and administer emergency medical treatment and care, including hospitalization, anesthesia, surgery, and other medical procedures, as they deem necessary for my well-being. I agree to bear all costs associated with any medical treatment provided. Release of Liability: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Philippi Church of Christ Disciples of Christ, its pastors, staff, volunteers, and any other individuals associated with VBS from any and all claims, demands, actions, or causes of action arising out of or related to any loss, injury, or damage that may occur during my participation in VBS, whether caused by negligence or otherwise. Photo and Video Release: I grant Philippi Church of Christ Disciples of Christ the right to use photographs and videos of me taken during VBS for promotional and educational purposes. I understand that these materials may be used on the church's website, social media platforms, and other promotional materials. Compliance with Rules: I agree to abide by all rules, guidelines, and instructions provided by the VBS staff and volunteers. I understand that failure to comply with these rules may result in my removal from the event.

By Checking this box I hereby give my consent for them to participate in VBS. I agree to all terms and conditions outlined in this waiver on their behalf.
